Before you start

You may be trying to describe a change in attention, mood, worry, or routines. A short questionnaire can help you notice a pattern and bring specific examples to a health care provider.

It can't determine whether you have a diagnosis or what treatment you need. Your circumstances, health, history, and preferences matter too.

    A printable questionnaire about depressive symptoms. It includes a question about thoughts of self-harm.

What to do with a result

Write down what felt accurate, what did not, and what you would want a provider to understand. You can bring the form or simply describe your answers.

A low score does not invalidate a concern, and a high score does not tell the whole story.
