CBT templates
Cognitive-behavioural worksheets applied to interpersonal, structural, and identity-based stressors. Use them in session, as between-session practice, or on your own.
How these templates work
Each worksheet explains what it is for, provides an example, and gives space to work through the process. You do not need to complete an entire collection. Start with the worksheet that most closely matches the question in front of you.
Choosing where to begin
- Activity diary if the week feels blurred or it is unclear where energy goes.
- Graded task assignment if a specific task feels too large to begin.
- Preparing for behavioural activation to rebuild pleasure, mastery, or routine.
- Thought record if a situation repeatedly triggers a strong conclusion or emotional response.
- Decisional balance if both changing and staying the same carry meaningful costs.
- Problem solving if the distressing thought is accurate and a decision is needed.
- Life chart to understand mood episodes across a longer period.
